Incident: Competitor forgot to lift harmonica holder

During the final solve of a competitor's 4x4 Blindfolded round, they forgot to life their harmonica holder to block sight from the cube. The competitor had already donned the blindfold and started to solve the puzzle. A delegate noticed this and was able to get a card in front of the competitor roughly 30 seconds into their solving stage.
The delegate deemed that the competitor was not using their error to their advantage and gave the competitor a provisional extra.
The WRC agreed to accept the original attempt and not the extra, basing the decision on how it is ultimately the judge's responsibility to ensure that the sight blocker is placed appropriately, as per B4c. Additionally, see B4c++.
